GDPR INFORMATION CLAUSE

for patients and clients of the Introspection Space

In accordance with Article 13, paragraphs 1 and 2 of the General Data Protection Regulation of April 27, 2016 (GDPR), we inform you about the principles of personal data processing:

1. Data Controller

2. Purposes and legal bases for data processing

Your personal data (including special category data concerning health) is processed for the following purposes:

Implementation of the therapeutic/development process: based on explicit consent (Article 9, paragraph 2, letter a of GDPR) and for the purpose of providing therapy services (Article 9, paragraph 2, letter h of GDPR).
Conclusion and performance of a contract: in connection with the use of services offered by the center (Article 6, paragraph 1, letter b of GDPR).
Maintaining documentation: to fulfill legal obligations, including accounting and tax obligations (Article 6, paragraph 1, letter c of GDPR).
Organizational contact: scheduling appointments, providing organizational information, which constitutes a legitimate interest of the Controller (Article 6, paragraph 1, letter f of GDPR).
Marketing activities: informing about the offer, workshops, and news – solely based on voluntary consent (Article 6, paragraph 1, letter a of GDPR).

3. Recipients of personal data

Data may be shared with entities supporting the operation of the center, with which the Controller has entered into data processing agreements, including:

providers of IT systems and applications supporting the process (e.g., appointment booking systems, Unyte application),
entities providing accounting services,
other specialists within the team (only to the necessary extent, maintaining professional secrecy).

4. Data retention period

Data related to the implementation of the therapeutic process is retained for the period specified by applicable legal regulations (depending on the nature of the service).
Billing and accounting data is retained for 5 years, counting from the end of the calendar year in which the tax payment deadline expired.
Data processed based on consent (e.g., for marketing purposes) – until it is withdrawn.

5. Rights of the data subject

You have the right to:

access your data and receive a copy of it,
rectification, deletion, or restriction of processing (within the limits set by regulations),
data portability,
withdraw consent at any time (without affecting the lawfulness of processing before its withdrawal),
object to processing based on the legitimate interest of the Controller.

6. Right to lodge a complaint

If you believe that the processing of your data violates GDPR provisions, you have the right to lodge a complaint with the supervisory authority – the President of the Personal Data Protection Office (PUODO).
